$10 Physical Day @ CCMS on May 25th
Attached you see the flyer for our Physical Day @ CCMS, which is on May 25th 3:30pm-5:30pm. It is $10 for the physical and will cover your athlete until the end of the 2023-24 school year. For the rising 9th grade parents, the physicals will be given to your athletes’ respective high school and we will keep a copy in case you need it also. This is a perfect way to have one less thing to worry about going into next school year. You can pick up packets at the front desk (starting Monday morning), which will have a hard copy of the physical along with expectations before your athlete can participate in workouts, tryouts, and practices. Please make sure to fill out the first two pages of the physical paperwork and send it back with $10 (cash or check) on May 25. Then all you have to do is wait for your athlete to call or text that they are ready on the 25th. Pick up will be in the car rider line.
We will also have Kona Ice ($2-$5) there for students to buy while they are waiting to be seen or after they have been seen and are waiting on a ride.
